Patient Forms

You can save time and expedite your first visit by downloading and completing the forms in our “New Patient Package” at home. Simply hand the forms to our receptionist at check-in. Also, remember to bring your insurance card and an ID with you to your first appointment.

What to Expect at Your First Visit

Your first visit will give you the opportunity to meet our team, learn more about orthodontics, receive a complete initial exam, and find out which treatment options will best meet your needs. During your initial consultation, we will:

  • Review your dental and medical history forms.
  • Provide a complete oral exam, with complimentary intra and extra-oral photographs and digital dental scans of your teeth and jaws, as needed, to determine not only if treatment is necessary but also to determine the best time to start treatment.
  • Discuss treatment options and help you create a customized treatment plan.
